Dear friends and colleagues!

I consider it necessary to congratulate all of us, our professional community, on the first small victory (achievement) - the restoration after a long break of the position of the chief specialist of the MHSD in nephrology (Order of the MHSD of 19.10.10). It was difficult work at different levels of government and health care management, in which many of us took part (Public forum in Kolomna, hearings in the Commission on Health and Social Policy of the Federation Council, discussion of the problem in the MHSD, discussion in the press). But this is just the first step that gives a chance to create a unified system of nephrological care within the framework of the healthcare reform being carried out in Russia. In order not to miss your chance - from the very beginning it is required to combine the efforts of all three nephrological societies.

From the available data and the opinions of fellow experts, one can imagine (one can imagine) the scale and depth of the problem that we are tackling. Despite a certain development of dialysis service and a slow increase in the number of places for renal replacement therapy (according to the RDO in 2008 - 142 per 1 million population, which is 5 times less than Europe, 10 - than in the USA and 20 - than in Japan ), “conservative” nephrological care is declining (reduced) - departments are closed, the number of beds is decreasing, there is no increase in the number of specialists.

One of the reasons is the lack of official statistics on the prevalence and mortality from renal diseases, which hides the true extent of the problem from policymakers and healthcare organizers - creating a false idea of \u200b\u200bthe low significance of renal pathology compared to the leading one in the existing healthcare model - cardiovascular. Meanwhile, in the United States, "nephritis" among the causes of death in 2000 took 9th place, the overall prevalence chronic illness kidney (GFR< 60 мл/мин с высоким риском сердечнососудистых катастроф) - 10% (что соответственно в России может составить до 14 млн человек). Отсутствуют также точные данные о материальных и кадровых ресурсах нефрологичес-кой службы - количестве и видах ЛПУ, оказывающих нефрологическую помощь, числе врачей-нефрологов.

The first important stage in the reconstruction of the unified nephrological service of Russia was the Working Meeting within the framework of the VII Congress of the NNR (21.10.10), the resolution of which outlined the priority measures.

The Order of the MHSD on the "Procedure for the provision of nephrological care ..." has been developed and posted on the website of the MHSP for analysis and discussion. The next step is the development and approval of "Standards" and "National Recommendations" on the most important problems of nephrology; for the preparation of each of the documents, "Working Groups" will be created from among the leading experts. After public discussion and revision, these documents will be approved at conferences, plenums or congresses of our scientific professional societies.

Shilov is one of the pioneers of Russian experimental immunonephrology, the author of studies devoted to the study of the mechanisms of development of immunoinflammatory and vascular diseases of the kidneys, the development, substantiation and evaluation of the clinical efficacy of programs for immunosuppressive therapy for glomerulonephritis, including in systemic diseases (systemic lupus erythematosus, vasculitis). In 1994 he defended his doctoral dissertation "Immunosuppressive therapy of active forms of nephritis (clinical and experimental study)", in which, based on the revealed differences in the effect of glucocorticosteroids and cyclophosphamide (CPA) on collagen deposition in kidney tissue, he experimentally substantiated the therapy ultra-high doses of CFA, which is still widely used in all nephrological centers in our country.

Today, under the leadership of E.M. Shilov, at the Department of Nephrology of the First Moscow State Medical University, new scientific directions have been formed: the study of various aspects (including genetic), vascular nephropathies, primarily thrombotic microangiopathies, the study of the features of kidney damage in metabolic syndrome, epidemiological studies in the field of chronic kidney disease. EAT. Shilov is the author of over 200 scientific works, including 2 monographs and 3 manuals on nephrology. Under the editorship of Evgeny Mikhailovich, the textbook "Nephrology" was published for the first time in domestic practice for the system of postgraduate professional training of doctors. Under the leadership of E.M. Shilov defended 3 doctoral and 15 master's theses.

EAT. Shilov was a member of the Presidium of the Scientific Society of Nephrologists of Russia, and in 2010 he was elected Vice President of the NNR; member of the Coordinating Council of the Russian Dialysis Society (RDO), is a member of the editorial boards of the journals "Nephrology and Dialysis", "Nephrology", "Clinical Nephrology".

12/27/2018 I underwent surgery to release the ulnar nerve on my left arm. The operation was performed with the golden hands of a very cool neurosurgeon Nasip Nuripashaevich Alipbekov. This doctor knows how to find an approach to each patient, easily makes contact in discussing treatment methods, takes into account the wishes of the patient. Moreover, he is fluent in the endoscope! In another hospital, they insisted on a strip operation with a 10 cm suture. In the Clinical Hospital No. 3, the First Moscow State Medical University named after I. Sechenov in the clinic of nervous diseases named after A. Ya. Kozhevnikova Alipbekov Nasip Nuripashaevich performed the operation with an endoscope, leaving the seam, only 2 cm. In the summer of 2018, I faced severe pain in my left leg and back. At first I thought I had a cold sciatic nerve, but after MRI it turned out that this is a herniated disc L4-L5. Conservative treatment in general, it helped to relieve severe pain, but after the treatment, the pain still remained, albeit in a muted form, until the next acute attack. After the third such attack, and there were three of them in 2 months, it became clear that it was necessary to turn to a neurosurgeon for help.
On the recommendation of a friend who was operated on here, in the neurosurgery department several years ago, and was very pleased with the treatment, I signed up for a consultation with the head of the department, Andrei Igorevich Rosen (my spouse went, since I myself practically did not move). After the consultation, Andrei Igorevich suggested that I go to the department for examination, which I did on 09/12/2018, having previously done all the medical examinations and tests necessary for hospitalization. I was sent to the attending doctor Alipbekov Nasip Nuripashaevich in ward 415. After studying my medical examinations and conducting additional, as well as repeatedly discussing my situation at the councils, the doctors diagnosed lateral stenosis of the spinal canal at the L4-L5 level and made a decision about surgical treatment... On September 17, 2018, I was operated on by N.N. Alipbekov, who performed decompression of the spinal canal at the L4-L5 level on the left, that is, removed the herniated L4-L5 intervertebral disc, which was compressing nerve root, which is why my left leg hurt very badly before the operation. The operation was successful and on the second day I got to my feet. 10/04/2018 I was discharged from the department and now I am going through the postoperative rehabilitation period. There are NO wild pains in my left leg that tormented me before the operation, and this fact is very pleasing! I express my HUGE gratitude to the entire team of the neurosurgical department for the highest professionalism and knowledge of their business, for attentiveness and care, for empathy and an individual approach to each patient!
Special thanks to the neurosurgeon who treated and operated on me - Alipbekov N.N.! Nasip Nuripashaevich is a talented, extremely able-bodied, sensitive, patient and very attentive doctor! A true professional in his field!
