pharmachologic effect

Viferon is a complex preparation containing recombinant human interferon alpha-2, ascorbic acid and alpha-tocopherol acetate. The drug has antiviral, immunomodulatory and antiproliferative effects. Interferon in the body enhances the activity of T-helpers, cytotoxic T-lymphocytes, increases phagocytic activity, the intensity of differentiation of B-lymphocytes. Interferon does not have a direct antiviral effect, but it causes changes in the cell affected by the virus and in the surrounding cells that prevent the virus from multiplying. It also promotes the release of viral particles from the affected cell, their inactivation by other immune agents. The drug also inhibits the replication and transcription of not only viruses, but also some bacteria, such as chlamydia. Damage to cell membranes, a violation of their permeability and elasticity, which irreversibly occurs during the development of an infectious process, leads to a decrease in the effectiveness of the action of interferon. Ascorbic acid and tocopherol acetate are powerful antioxidants and membrane stabilizing components. In combination with them, the effectiveness of interferon increases 10-15 times. In addition, their presence enhances the immunomodulatory effect of interferon on T- and B-lymphocytes, leads to the normalization of the content of immunoglobulin E, reduces the manifestation side effects... When using Viferon, even for a long time, antibodies are not formed that neutralize interferon.
The use of the drug can significantly reduce the dose and duration of treatment with antibiotic therapy, hormonal drugs and cytostatics.

A decrease in the concentration of interferon in the blood with rectal administration occurs
12 hours after application, which is much longer than with intravenous administration interferon, and necessitates re-introduction the drug Viferon after 12 hours. In premature infants with a gestational age of less than 34 weeks, the drug is excreted faster, and the need for re-administration occurs 8 hours after using the drug.

Indications for use

Suppositories:
Complex therapy of various infectious and inflammatory diseases in children and adults, including acute respiratory viral infections (influenza, diseases respiratory tract complicated bacterial infection), pneumonia (viral, bacterial, chlamydial), meningitis (bacterial, viral), sepsis. Intrauterine infections, including chlamydia, herpes, cytomegaly, candidiasis (including visceral), mycoplasmosis.
Complex therapy of hepatitis B, C and D, in children and adults, including chronic viral hepatitis of a pronounced degree of activity and complicated by liver cirrhosis.
Complex therapy of urogenital infections in adults, including pregnant women (chlamydia, cytomegalovirus infection, ureaplasmosis, trichomoniasis, gardnerellosis, papillomavirus infection, bacterial vaginosis, recurrent vaginal candidiasis, mycoplasmosis). Herpetic infection, including the genital form.

Ointment:
Herpetic lesions of the skin and mucous membranes, papillomavirus infection (vulgar warts, genital warts, bowenoid papulosis).

Mode of application

Rectal suppositories:

Repeated courses are possible with various diseases, but the break between courses must be at least 5 days. In children, including premature babies, repeated courses are carried out with sepsis 2-3 courses, with herpes infection 2 courses, cytomegalovirus infection, mycoplasmosis, candidiasis 2-3 courses. In case of acute respiratory viral infections and pneumonia, by decision of the doctor, a second course of treatment with Viferon can also be carried out.

With the complex therapy of chronic viral hepatitis in children and adults:
Children, depending on age, are prescribed: up to 6 months - 300000-500000ME per day; from 6 to 12 months - 500000 ME per day; from 1 to 7 years - 300000 ME per 1 m2 of body surface area per day. Children over the age of 7 years - 500000 IU per 1 m2 of body surface area per day. The dose of the drug is divided into 2 doses with a break of 12 hours for the first 10 days daily, then three times a week every other day for 6-12 months.
Adults are prescribed Viferon 3000000ME 1 suppository 2 times a day with a break of 12 hours for the first 10 days daily, then three times a week every other day for 6-12 months.

With the complex therapy of urogenital infections, including in pregnant women:
Adults are prescribed Viferon 500000ME 1 suppository 2 times a day with a break of 12 hours. The course is 5-10 days. According to the indications, an additional course can be carried out after a 5-day break. In case of herpes infection, Viferon 1000000ME is prescribed, 1 suppository 2 times a day with a break of 12 hours. The course of treatment is 10 days. It is advisable to start treatment at the first signs of the development of the disease.
For pregnant women (from 14 weeks of pregnancy) Viferon 500000ME 1 suppository 2 times a day every 12 hours every day for 10 days, then 1 suppository 2 times a day twice a week for 10 days, then after 4 weeks it is necessary to carry out a prophylactic course of Viferon 150000ME 1 suppository 2 times a day for 5 days. Complex therapy of infectious diseases in adults: Viferon 500000ME is prescribed 1 suppository 2 times a day after 12 hours. The course of treatment is 5-10 days.

Ointment:
Apply in a thin layer to the affected areas of the skin 3-4 times a day and rub in gently. The course of treatment is 5-7 days.

Side effects

Suppositories: in isolated cases, the development of skin allergic reactions, such as itching and hives, which disappear within 72 hours after drug withdrawal.

Contraindications

The use of suppositories makes it possible to stop a viral infection when its first symptoms appear or to completely prevent its penetration into cells (that is, suppositories can be used to prevent the disease).

Suppositories 150,000 IU are used in children under 7 years of age, including newborns and premature babies. The drug is prescribed for babies born at a period of less than 34 weeks for 1 candle 3 times a day, for all other children 2 times a day.

Suppositories 500,000 IU are prescribed for children over 7 years old, 1 suppository 2 times a day.

The duration of therapy is 5 days, at the discretion of the pediatrician, the treatment regimen can be extended.

For infections in newborns born more than 34 weeks, suppositories 150,000 IU are prescribed 1 suppository 2 times a day, premature babies born before 34 weeks - 3 times a day for 5 days.

The number of courses for infections in newborns is as follows:

• Blood infection, mycoplasmosis, cytomegalovirus infection - 2-3 courses
• Inflammation of the meninges, - 1-2 courses

For chronic hepatitis B, C, D, the drug is prescribed:

• infants under six months in a daily dosage of 300,000-500,000 IU;
• infants aged 0.5 to 1 year in a daily dose - 500,000 IU;
• for patients aged from one to 7 years in a daily dose of 3,000,000 IU per 1m 2 of body surface area;
• for patients who have reached 7 years of age in a daily dosage - 5,000,000 IU per 1m 2 of body surface area.

Candles are inserted 2 times a day for 10 days, then 3 times a week every other day for six months to a year. The duration of therapy depends on the well-being of the child and the results of laboratory tests.

The daily dosage of the drug is calculated for each child by multiplying the recommended dosage for a given age group by the body surface area calculated from the Graford, Terry and Rourke nomogram. In order to calculate a single dosage, the daily dose is divided in half, and the result is rounded up to the dose of the candle.

Children suffering from chronic aggressive viral hepatitis and liver cirrhosis are prescribed in the following dosages before plasma cytophoresis and hemosorption:

• children under 7 years of age are prescribed suppositories 150 000 IU;
• patients over 7 years old - 500,000 IU.

Candles need to be inserted rectally 2 times a day for 2 weeks.

Side effects

Adverse reactions to suppositories are very rare. This can be a rash around the anus and / or itching. If you suddenly notice such manifestations in a child, you should cancel the drug. Symptoms go away on their own within 3 days.

There were no cases of severe allergic reactions with the use of suppositories.

Interaction with other medicinal products

Suppositories go great with all drugs, which are used in the treatment of diseases indicated in the list of its indications.

They are prescribed as an addition to the main therapy; they give good results in combination with antibiotics, and with hormonal drugs, and with medicines of other pharmaceutical groups.

Indications for use

Viferon in the form of an ointment is prescribed for the treatment of the following diseases:

The preparation in the form of a gel is indicated for use:

• SARS, including influenza, long-term and frequent SARS, incl. proceeding with complications of bacterial infections (prevention, simultaneously with other drugs);
• Recurrent stenosing laryngotracheobronchitis (prophylaxis, along with other drugs);
• Herpetic cervicitis (along with other drugs);
• Herpetic infection of the mucous membranes and skin (acute and exacerbation of chronic recurrent), incl. urogenital form of herpes infection (simultaneously with other drugs).

Viferon in the form of suppositories is used simultaneously with other drugs in the treatment of diseases:

• SARS, including influenza, incl. proceeding with complications of bacterial infections, pneumonia (chlamydial, viral, bacterial) in adults and children;
• Chronic viral hepatitis B, C, D in adults and children, incl. in combination with the use of hemosorption and plasmapheresis in chronic viral hepatitis of pronounced activity, which are complicated by cirrhosis of the liver;
• Infectious and inflammatory diseases in newborns (including premature babies): sepsis, meningitis (viral, bacterial), intrauterine infection (enterovirus infection, chlamydia, CMV infection, herpes, mycoplasmosis, candidiasis, including visceral);
• Recurrent or primary herpes infection of the mucous membranes and skin, localized form, moderate and mild course, including the urogenital form in adults;
• Infectious and inflammatory diseases of the urogenital tract (bacterial vaginosis, ureaplasmosis, chlamydia, papillomavirus infection, CMV infection, gardnerellosis, trichomoniasis, mycoplasmosis, recurrent vaginal candidiasis) in adults.

Contraindications

Hypersensitivity to the components of the drug.

Method of administration and dosage

Viferon ointment is used topically and externally.

Herpetic infection: the ointment should be applied 3-4 times a day with a thin layer on the lesions and rubbed gently. The duration of therapy is from 5 to 7 days. It is recommended to start treatment when the first signs of the disease appear (redness, burning, itching). With recurrent herpes, it is advisable to start treatment in the prodromal period or when the first signs of relapse appear.

Influenza and other acute respiratory viral infections: the ointment is applied in a thin layer to the mucous membrane of the nasal passages 3-4 times a day during the entire period of the disease. Frequency rate for children 1-2 years old - 3 times a day, 2-12 years old - 4 times a day.

Gel Viferon is used topically and externally.

Complex therapy of ARVI: gel (a strip up to 5 mm long) should be applied to the surface of the nasal mucosa, which must be pre-dried, and / or to the surface of the tonsils using a spatula or cotton swab, 3-5 times a day. The duration of the treatment course is 5 days, if necessary, the therapy can be continued. For the prevention of acute respiratory viral infections during the rise in incidence, a strip of gel up to 5 mm is applied 2 times a day. The duration of Viferon's use is 14-28 days.

Complex therapy of recurrent stenosing laryngotracheobronchitis: a strip of gel up to 5 mm long should be applied to the surface of the tonsils. In the acute period of the disease (first 5-7 days) - 5 times a day, then within 21 days - 3 times a day. To prevent the disease, the gel is used for 21-28 days 2 times a day, it is recommended to repeat the courses 2 times a year.

Complex therapy of acute and chronic recurrent herpes infection (when the first signs of the disease appear): a strip of gel up to 5 mm should be applied with a cotton swab / cotton swab or spatula on the affected surface, previously dried, 3-5 times a day for 5-6 days. The duration of the course can be increased until the disappearance of clinical manifestations.

Complex therapy of herpetic cervicitis: 1 ml of gel with a cotton swab should be applied 2 times a day to the surface of the cervix, which must first be cleared of mucus. The duration of the drug use is 7-14 days.

The gel should be applied to the surface of the palatine tonsils half an hour after eating, on the nasal mucosa - after cleaning the nasal passages. When Viferon is applied to the affected areas of the mucous membranes and skin, a thin film is formed within 30-40 minutes, on which the next dose is subsequently applied. If desired, the film can be washed off with water or peeled off.

Viferon suppositories are used rectally.

Complex therapy of acute respiratory viral infections:

• Adults and children from 7 years old - every day for 5 days (according to indications, possibly longer). 2 times a day with a break of 12 hours, 1 suppository 500,000 IU;
• Children under 7 years of age, including newborns and premature infants with gestational age of 34 weeks - 1 suppository daily 150,000 IU 2 times a day with an interval of 12 hours for 5 days (if indicated, treatment can be continued with an interval of 5 days);
• Premature infants with gestational age up to 34 weeks - 1 suppository of 150,000 IU daily, 3 times a day with an interval of 8 hours for 5 days (if indicated, treatment can be continued with an interval of 5 days).

Complex therapy of infectious and inflammatory diseases (sepsis, meningitis, intrauterine infections, including CMV infection, chlamydia, herpes, candidiasis, enterovirus infection) of newborn children, incl. premature babies with gestational age: over 34 weeks - daily 1 suppository 150,000 IU 2 times a day with an interval of 12 hours, up to 34 weeks - daily 1 suppository 150,000 IU 3 times a day with an interval of 8 hours. The treatment course is 5 days.

• Sepsis - 2-3 courses;
• CMV infection - 2-3 courses;
• Herpetic infection - 2 courses;
• Meningitis - 1-2 courses;
• Mycoplasmosis, candidiasis, incl. visceral - 2-3 courses;
• Enterovirus infection - 1-2 courses.

In the presence of clinical indications, treatment can be continued.

Complex therapy of chronic viral hepatitis B, C, D: 10 days Viferon is used daily 2 times a day every 12 hours, then 6-12 months - 3 times a week every other day. The duration of therapy is determined by laboratory parameters and clinical efficacy. Daily dose is:

• Adults - 2 suppositories of 3,000,000 IU each;
• Children from 7 years old - 5,000,000 IU per 1 m2 of body surface area;
• Children 1-7 years old - 3,000,000 IU per 1 m2 of body surface area;
• Children 6-12 months - 500,000 IU;
• Children under 6 months - 300,000-500,000 IU.

Chronic viral hepatitis of pronounced activity and cirrhosis of the liver: Before hemosorption and / or plasmapheresis, children under 7 years old are recommended to use Viferon daily at 150,000 IU, children over 7 years old - 500,000 IU 2 times a day with a break of 12 hours for 14 days.

Complex therapy of infectious and inflammatory diseases of the urogenital tract (mycoplasmosis, ureaplasmosis, papillomavirus infection, chlamydia, CMV infection, gardnerellosis, trichomoniasis, recurrent vaginal candidiasis, bacterial vaginosis): adults - 5-10 days daily for 1 suppository of Viferon 500,000 IU. Frequency rate of application - 2 times a day (every 12 hours). In the presence of clinical indications, treatment can be continued. According to the same scheme, in the first 10 days of treatment, Viferon is prescribed to pregnant women after 14 weeks of gestation. In the next 10 days, the drug is prescribed 1 suppository 2 times a day with a break of 12 hours every 4th day. Further - every 28 days until delivery, 1 suppository 150,000 IU 2 times a day with the same interval daily for 5 days. If necessary, before delivery (from 38 weeks of gestation), 1 suppository 500,000 IU 2 times a day is prescribed for 10 days.

Recurrent or primary herpes infection of the skin and mucous membranes, localized form (moderate and mild): the recommended single dose for adults for 10 days is 1,000,000 IU, for pregnant women from the 2nd trimester - 500,000 IU. The drug is used daily, 2 times a day (every 12 hours). In the presence of clinical indications, therapy can be continued. In the future, Viferon can be used for pregnant women according to the treatment regimen for infectious and inflammatory diseases of the urogenital tract.

Side effects

Viferon in the form of an ointment is well tolerated in most cases. Side effects when applied to the nasal mucosa are usually transient and mild and disappear on their own after discontinuation of the drug.

In extremely rare cases, when using the gel in patients with hypersensitivity, local allergic reactions may develop. When they appear, treatment is stopped.

When using Viferon suppositories, in some cases, the development of allergic reactions (itching, skin rashes) is possible. These phenomena are reversible and disappear 72 hours after the cessation of therapy.

Human recombinant interferon alpha-2 has pronounced antiviral, immunomodulatory and antiproliferative properties. Complex composition the drug causes a number of additional effects: in the presence of ascorbic acid and alpha-tocopherol acetate, the antiviral activity of human recombinant alpha-2 interferon increases, its immunomodulatory effect on T- and B-lymphocytes increases, the level of immunoglobulin E is normalized, and the functioning of the endogenous interferon system is restored. In addition, ascorbic acid and alpha-tocopherol acetate, being highly active antioxidants, have anti-inflammatory, membrane stabilizing and regenerating properties. It was found that when using VIFERON there are no side effects that occur with parenteral administration of interferon preparations, and antibodies are not formed that neutralize the antiviral activity of interferon.

Pharmacokinetics

After rectal administration, after 12 hours, a decrease in serum concentration is observed, which necessitates its repeated administration.

Indications for use
