Hello. 05/03/2018 in the children's clinic on Engels Street, an outrageous, in my opinion, case occurred. Today we decided to contact the pediatrician of the 8th site, because a 5-year-old child fell ill, the first day his temperature was 38 and his throat was red. I want to immediately note that the child does not tolerate the temperature well, a couple of years ago the syndrome was transferred twice febrile seizures against a background of high temperature. So, when we turned to the site for help, the appointment was coming to an end, or rather, there were 40 minutes left until the end. Due to the fact that there were still a lot of people, I nevertheless decided to wait until the end of the appointment, hoping that the doctor would still accept the little patient with fever. And what was my indignation, or rather, disappointed when the pediatrician, loudly and clearly saying that he would not accept us, got up from the table, and despite my request, based on the fact that only I was left with a child who had a fever , and it will be difficult for him to sit in a long queue for another section, calmly and silently gathered her things and, not paying attention to us, left the office. This behavior made me uncomfortable, well, how is it possible, because this is a pediatrician, and if a child comes to the doctor for help, it’s not just that. All this happens during a period of unfavorable epidemiological situation. As a result, I took the child by the hand and left the clinic, because it was impossible to get to another area, and the child wanted to sleep, the temperature was 38. I really want to believe that, after all, my cry from the heart will be heard by the leadership of the pediatric department, and by herself a doctor, because she probably also has grandchildren.

I went in the summer for a consultation about embolization. Some kind of shaggy redhead manager. department, looking at me, asked about my weight. Then she expressed doubts about the operation for the reason: "What will the surgeon dig into your fat?" Together with their sucker / deputy or girlfriend / they staged a performance in the spirit of "we are not smarter in the world", prescribed some hormones, and if after six months they do not help, then extirpation. The background to all this is as follows. I was sent from a small town for a consultation, I'm 45, I work as a dispatcher. The doctor said to call the Khanty and ask if they would be accepted with such a diagnosis. I called, the phone was picked up by a weak-speaking doctor and asked who you are. I explain the situation, throws the pipe. I call again, again the same thing. She is not only weakly speaking, apparently, she has it system-wide. By the nature of my activity, I do not understand how you can send a person without resolving his situation, even just with advice. Almost the same I told her. As a result, the doctor spoke, my doctor also signed me up. Apparently the weak-speaking doctor remembered my appeal to her. I arrived in Khanty, I'm waiting for an appointment, some kind of running around, looking askance. Well, they are with a redhead and broke away from the soul, as they say, on the woman's side. I have long wanted to write a review, but now I’ve got my hands on it. All health.

I am in the neurosurgical department for the second time and I want to express my gratitude to all the staff headed by E.S. Kolesnikov. Very knowledgeable and helpful people.

How to get the coveted coupon?

Morning assault. This is how the residents of Khanty-Mansiysk now call an appointment to see a doctor in the morning. Every morning at the terminal in the hall of the OKB, the sick gather in order to get the coveted ticket to a specialist in battle.

“I saw recently how the crowd in the OKB almost fought for recording on the terminal,” says resident of Khanty-Mansiysk Victoria Koloznikova... - The line of aggressive patients began already at the entrance to the hospital, moreover, not only grandmothers stood in it, give them a reason to create a queue somewhere, but also young people. People pushed each other with their elbows, did not allow to pass to the terminal ”.

At the same time, the girl herself came to the hospital for exactly the same assault in the children's clinic, but was among the losers. She needed to show her son to an otolaryngologist, for which there is no record in the electronic registry. Victoria was helped by ... a cleaning lady who let her into the building of the OKB branch the next morning.

“They let me get warm. In the corridor I signed up at the terminal myself. But it was like a computer game "Manage to get into the green square", - said the girl.

Many patients associate the sudden shortage of appointments with a change in the work of the district electronic registry, with the help of which they used to make appointments without problems. But now this can be done only through authorization on the State Services portal.

"Does not work. The tablet constantly freezes, - complains resident of the district capital Anastasia Kuznetsova.- Last week I tried to make an appointment for my daughter to see a neurologist, I see a free window at the doctor's on the screen, but I can’t “clog” it. I called the registry and asked to register my daughter there. "

As it turned out, the Registry86.rf website continues to operate as before. Ugra residents only need to create a personal account here, of course, with authorization on the State Services portal. This information was not enough for many patients to make an appointment without any problems. By the way, master State portal, sooner or later, all residents of Russia will have to, since duplicate regional sites are likely to be closed as unnecessary.

However, some experts do not consider electronic recording a panacea, rather the opposite. So, according to members of the Popular Front in Ugra, it is thanks to the Internet recording in the district that the problem of the lack of an appointment with narrow specialists is artificially created. Often, patients come to doctors who have diagnosed themselves and appointed a doctor. At the same time, according to statistics, after self-registration, up to 800 residents of Ugra do not come to an appointment every month. ONF experts believe that only a therapist should refer to a specialist.

    When will registries disappear?

According to the data of the regional health department, the indicator of provision of the population with doctors of all specialties (per 10 thousand Ugra residents) is 50.3. Whereas in Russia this bar does not even reach 38. A similar situation with the average medical staff: in Ugra - 141.4, in the country - about 90.

Today, you can get an appointment with a doctor in the district in different ways: make an appointment by phone, via the Internet, at the terminal, at the reception and on a first-come, first-served basis.

“Our patients use every route. In a children's clinic, they are more often registered through an electronic registration, in an adult - by phone, '' says Chief Physician of the Surgut Polyclinic Maxim Slepov... According to him, there is a category of citizens who literally live in hospitals. More often they are lonely elderly people who feel attention deficit. “Hospitals are their home, and we cannot refuse them according to the law, although such“ patients ”take time and resources."

Our interlocutor notes that today the Surgut registries got rid of the constant queues. Experts and chief physicians can make sure of this at any time, because online video cameras are installed above the "information desks". Employees of the medical information-analytical center of Ugra review these records as part of the monitoring of the work of the registries.

By the way, doctors consider these departments an anachronism, which will soon be unnecessary. This is what the health care industry in Ugra strives for. Once federal legislation is finalized and allows hospitals to get rid of paper medical records, registries will sink into oblivion. They will be replaced by hall administrators who will simply help to record through the terminal or redistribute patient flows.

Moreover, Ugra medicine is heading towards the fact that soon some patients will not even have to wait in line to see a doctor. On the portal of the modernized electronic registry in the updated personal account, patients will be able to receive specialist advice without getting up from the couch. To do this, the user will only have to provide access to his data to the selected specialists, and they, in turn, having reviewed the results of examinations and complaints, will issue a verdict. The only stumbling block now for the introduction of such technologies is legal issues related to the observance of medical confidentiality and personal data of patients. From a technical point of view, everything is ready in Ugra, but the legislation at the federal level is still lagging behind.

But this is the future, and today some Ugra residents continue to storm the terminals, and the high regional indicators of the availability of medical care do not comfort them much.

    Officially

To date, 43.5% of the initial appointment is made using remote mechanisms, such as Federal and regional electronic registries, websites medical organizations, health care contact center and self-service terminals. More than one million appointment services are provided electronically, almost half of which are provided through the Federal and Regional Electronic Registry.

In 2017, in order to improve accessibility medical care On the basis of the regional registry, a mechanism was developed and tested that will allow patients, in the absence of an available time for a doctor's appointment, to leave an application for an appointment, indicating the appropriate time, after which the registrars of medical organizations will contact the patient and select the most comfortable appointment time. The service was named the "Waiting List" and will be launched in pilot mode in December 2017, after which it will be replicated throughout the district in 2018.

To popularize the State Services portal, since August 2017, a remote appointment with a doctor is provided exclusively to patients authorized using the portal account, which made it possible to increase the involvement of citizens in receiving electronic health services.

Expert of the ONF in Ugra, deputy of the Surgut Duma Maxim Slepov:

    - As a social activist, I participated in the work of experts who were assessing the availability of medical care in the districts of the Okrug. They sat in the corridors of hospitals and wrote down the time of the appointment, tried to make appointments with doctors in different ways, and so on.

As a result, we came to the conclusion that medical care is really available in the districts, however, each particular municipality has its own shortcomings. These are working moments, which will always be - somewhere the registration is uninformative, somewhere the "decoy" patients could not make an appointment via the Internet, sometimes the hospital administration did not make contact.

But in general, our district is in the lead in Russia in terms of the availability of medical care. You can sign up anywhere and anytime.

    By the way:

OKB Khanty-Mansiysk is the only district clinic that provides comprehensive services for primary, specialized and high-tech assistance, that is, there are both inpatient departments and polyclinics: adult, children's and women's consultation. Every year, the OKB polyclinics receive 700 thousand patients.

My 2-year-old daughter and I came to planned hospitalization from the city of Urai on 01.12.2011 for making a diagnosis. We were admitted and registered in the pediatric department. We were in the ward at about 2 pm, given that we were driving from Urai to Khanty-Mansiysk for 6 hours (on the road from 7 am), the doctor examined us only at about 10 pm after my 3 request, the attending physician is with us was the head of the department Bostanova A.B. 02.12.2011 passed the tests. 03 and 04 December were days off. On December 05, on Monday, a doctor examined us at about 11 o'clock, they did not do anything else with us that day, as well as on weekends. On the night of December 5-6, at about 3 am, the child began to have a dry, frequent cough with yellow vomiting. I took the baby in my arms (given that the baby weighs 17 kg and I am 7 months pregnant) went to the staff room and asked the doctor on duty to examine us. After listening to us, the doctor said that everything was fine, at my request to look at the child's throat, I heard that she did not know where they were lying

spatula and generally go to the ward, you will not do anything until morning. I went with the child in my arms to the ward, she was coughing up with vomiting, she only had time to carry it to the toilet, she was all in tears from such help, there was a desire to walk home to Uraya at night. Finally it came 7 o'clock in the morning on December 6, a m / s came to us with thermometers, I asked her when the doctor would come to us, about 15 minutes later the same night doctor came to us, after listening to us she said that everything was fine, asked how many times she vomited until morning , to which I replied that I did not count, because this process was almost every 5-10 minutes. The doctor on duty left, the m / s came with a syringe, to the question "what is this?" m / s said that this antiemetic agent "circus", I told her that for a start, let the doctor look at our throat. The doctor came, looked at the throat, said that everything was fine, I say okay put "Cirucal". M / s came, bent down to give an injection, and she smelled of fumes, I also thought, "I got a job successfully." department marked some event in the office of the head. department, this was all witnessed by my roommate. On the morning of December 6, I went to our attending physician head. department, in the office there was a terrible smell of fumes, I told what happened at night with my child, to which she replied, I will come after the planning meeting. The time is 10 o'clock in the morning, an epidemiologist from the senior medical officer came to us, they began to question what they ate and drank in the evening, how many times we vomited at night, I told them what they ate and drank, and left. At about 11:30, our doctor came to us, apparently the planning meetings were over, she listened to us, she said we had to get tested for CITO. At about 12 o'clock the temperature rose to 38, I informed the doctor about this and said that we were vomiting even from the water. I asked the doctor to give us a referral for rotovirus. After lunch, we donated blood, so far there has been no more help. Once again asking about the results of the blood, the doctor told me that the analysis was ready, but she still had no time to look. Yet again, once again going to the doctor, asking about the results, the doctor said that our leukocytes were elevated, it turned out that not only we are like that, but 2 more children with the same condition, one child was transferred at night to the infectious diseases department. At about 6 pm we finally got an antibiotic. Thanks to this attitude and treatment with us, my stomach began to hurt, I could hardly walk down the corridor from the doctor (asking the results) to the ward, when she saw me, one of the nurses asked “are you bad?”, I said “yes,” she called to me therapist. The therapist looked at me, called the gynecologist from the pathology department. And then it all started: they were running around, fussing. The gynecologist examined me, hooked up the apparatus, measured the tone, did an ultrasound scan, took blood for tests. The nurses and medical attendants began to run to our ward, and even our attending physician came to the ward several times during the evening and asked how I felt and your daughter's temperature? I refused to be admitted to the pathology department, because Seeing the attitude of doctors and m / s towards us, I was afraid to leave the child in the ped. department one, I was simply amazed at the indifference of the head. departments, doctors and individuals m / s. With pathology, the midwife gave an injection and an IV, after which I calmed down a little, but tears just rolled by themselves, from the fact that we were in another city, we have no one here, and this is the attitude, although I drove with the hope that at last we would be diagnosed , why and why do we have a cough, and why does the child have a terrible smell of acetone from the mouth? At about 10 pm, the daughter was given another antibiotic. The night seemed to have passed calmly, without vomiting, but with a temperature of 37-37.5 C. On December 7, the tests were repeated, my daughters were given an antibiotic, I had an IV and an injection, and they also took blood for tests. The day passed calmly, without vomiting, with the temperature rising. On the day of departure, December 8, I was reprimanded as a schoolgirl m / s, for looking at my extract with analyzes from the pathology department, an outpatient pregnancy card that I told
